Pharmacy Clinical Services Manager

The Role:

The Manager of Clinical Pharmacy Services is responsible for development, oversight and optimization of clinical pharmacy services and initiatives across CCC.  As a member of the CCC clinical leadership team, you will help to shape CCC’s Quality & Safety Plan, Population Health Strategy and Clinical Standards.  Collaborates with medical and operations leadership to develop successful clinical pharmacy services and population health-based initiatives (e.g., Hepatitis C, Diabetes, Hypertension, Dyslipidemia, PreP, Asthma, and COPD).  Leads formulary management and stewardship and serves as a key member of CCC Quality, Clinical Standards and Sentinel Event committees. Provides educational and developmental opportunities for the clinical pharmacy staff and residents.

Job Scope:

Leadership

  • Develops and leads contemporary, evidence-based clinical pharmacy programs and services to support CCC’s Quality & Safety Plan and Population Health Strategy
  • Accountable for the strategic planning, business model, clinical standards and performance of clinical pharmacy services
  • Assists the Sr. Director of Pharmacy in planning, monitoring and/or managing budget for Clinical Pharmacy Services
  • Collaborates with medical leadership and other providers to develop practice standards and therapeutic algorithms
  • Coordinates relationship with Colleges of Pharmacy and provides oversight for Clinical Pharmacy rotations to ensure a positive learning experience and patient safety.
  • Collaborates with the Chief Medical Officer to enable a strategy for evidence-based clinical education for CCC providers, pharmacists and staff aligned with performance goals and safety practices
  • Supports the Sr. Director of Pharmacy to Coordinate organizational response to urgent and emerging medication related issues
  • Participates in various CCC project or improvement teams to represent and promote clinical pharmacy services through active leadership

Clinkical Pharmacy Services 

  • Develops short and long-term strategies to leverage clinical pharmacy services in response to organizational goals and national healthcare trends
  • Assures excellent teamwork and communication between clinical pharmacy services and other health center teams as well as support functions
  • Assures that clinical pharmacy services are delivered in accordance with the laws, rules, and regulation of the Oregon Board of Pharmacy, Department of Public Health, and the Food and Drug Administration and the Drug Enforcement Administration
  • Supports the Sr. Director of Pharmacy to Coordinate organizational response to urgent and emerging medication related issues
  • Participates on various CCC project or improvement teams to represent and promote clinical pharmacy services through active leadership

Pharmaceutical Standars and Stewardship

  • Under the direction of the Sr. Director of Pharmacy and the Chief Medical Officer, coordinates CCC’s P&T Committee functions.
  • Collaborates with the Sr. Director of Pharmacy to monitor and report trended performance in pharmacy utilization and medical adherence
  • Collaborates with interdisciplinary care team members to promotes a philosophy and activities to achieve evidence-based, cost-effective medication prescribing
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee

Requirements:

You will have an opportunity to lead and contribute to unique projects in this role, including:

  • Doctor of Pharmacy degree from an accredited pharmacy school (or equivalent degree) and licensed in the State of Oregon
  • Completed an ASHP accredited pharmacy residency (preferred)
  • Three (3) years of experience managing clinical pharmacy services in an integrated health delivery system setting
  • Current CPR/BLS certification
  • If in recovery, must be able to document continuous abstinence under independent living conditions or recovery housing for the immediate past two years
  • Must pass a pre-employment drug screen, TB test, and background check
